Create a Basic New Form (Quick Setup)
Use this Guide to create a quick form with basic features.
Log into your dashboard and select Features > Forms
Select +New
Here you will specify your form settings:
Upload your Form Icon Image. Select Library to choose from a prepopulated selection of icons. We recommend the ios7 icons.
Name your form.
Add one or more email addresses of response recipients.
Select Ok
Build Your Form
Locate the form you are building and select the Form Builder icon to the right.
Below you will find the Component Tip Sheet and two examples of component use.
Component Tip Sheet:
Input Field - a simple blank field for the user to type in an answer
Fill in the Blank - to create blanks in a sentence that need to be filled by the user
Radio Buttons - multiple choice answer; allows for user to select one answer
Address - an address field; Street Address, City, State, Zipcode
Signature - a digital signature field that users can use their finger to sign
Paragraph - to enter a long paragraph of information for users to read
Checkboxes - checkboxes for users to select from; users can select multiple answers
Dropdown - a dropdown menu of answers for users to select from; users can select one answer
Attachment - a place for users to attach files
Example 1: Adding a question with checkboxes
Select Checkboxes
The label field can be used for your question or for the subject of the question. The description field can be used for your question or for instructions on answering the question. These two options are versatile to meet your specific needs.
Type your answers in the Add Option field. For each additional answer click the plus sign.
Once all of your answer options have been entered, select the check mark to move to the next component selection.
Example 2: Adding questions with a dropdown selection

Once all of your answer options have been entered, select the check mark to move to the next component selection.
6. After you have completed your form creation, you can view a Live Preview by selecting the icon at the top of the page.
Form changes are auto-saved. If you would like to be sure your changes have been saved before exiting, select Changes Saved at the top of the page.
